In order to be considered for an account with German Auto Parts, Inc. you must be one of the above cited organizations and you must submit an application to us using the following forms:

Credit Application
A credit application is required along with the contact information form. Complete all sections of this from and provide the required personal guarantee signature(s) on this form. No accounts will be opened until a signed original of this form is on file with German Auto Parts, Inc.

Sales Tax Exemption for Georgia Businesses (only)
Georgia State Law requires that we have on record for all our Georgia Customers the attached ST-5 form for sales and use tax. Please fill in the necessary information and return this form to our receivables department.
